PLEASE READ THIS DOCUMENT CAREFULLY.

By visiting any ME SPA location, you acknowledge and accept our terms and conditions. If you do not agree to be bound by these terms and conditions, you may not access our spa or spa services. 

Release and Consent

You accept full responsibility for the service(s) rendered at Me Bath Spa Experience, LLC and its affiliates (DBA ME SPA) and release ME SPA of any and all liability for any possible outcomes, whether known or unknown, foreseen or unforeseen. You acknowledge and agree that you are responsible for your own health, and that ME SPA therapists are not health care practitioners and cannot be expected to diagnose and/or treat individual health issues. You understand that you are responsible for discussing any questions that you may have concerning your health conditions (if any) during the course of your treatment. By voluntarily choosing to receive spa treatments, you warrant that to the best of your knowledge, you have no condition, impairment or ailment that prevents you from receiving such treatment(s). You hereby forever release, covenant not to sue, discharge, and hold harmless ME SPA (and its parent corporation, subsidiaries, affiliated corporations, and their respective officers, directors, shareholders, employees, and successors) and waive any and all claims, causes of action, injuries, liabilities, damages, costs, and expenses of any kind, including claims arising from the ordinary negligence of ME SPA that you may experience directly or indirectly in connection with your attendance at ME SPA or participation in ME SPA services.

Consent Forms:

Upon booking your appointment or your arrival at the spa, the following forms are required and you are responsible for providing accurate contact and health information. 

  • Release & Consent Form- You acknowledge that the information you provide on your Release and Consent Form is true and accurate, and you agree to all terms and conditions in your Release and Consent Form and understand that you are waiving valuable legal rights. If any changes are made to the information you have submitted on that form, you agree to verbally disclose those changes to my therapist. YOU AGREE THAT YOUR RELEASE AND CONSENT FORM SHALL REMAIN IN EFFECT FOR FUTURE VISITS AND SHALL AUTOMATICALLY APPLY TO EACH SUBSEQUENT VISIT UNLESS AND UNTIL UPDATED OR REVOKED IN WRITING.

  • Peel/HydroDermabrasion Form: to be filled out for each visit, no exceptions.

  • Minor Form: to be filled out by a legal guardian for minors age 12-17.  

Arbitration Policy

You acknowledge that the exclusive method for resolving any controversy, dispute, or claim arising out of or with respect to my dealings with ME SPA shall be by Binding Arbitration. Such arbitration shall be before and in accordance with the rules and regulations of the American Arbitration Association. Any arbitration shall be held in Paradise Valley, Arizona, and the parties hereby waive any objections that they may have based on improper venue or foreign non conveniens. In addition, you and ME SPA each agree that any disputes shall be arbitrated only on an individual basis and not as a plaintiff or a class member in any purported class, representative proceeding or as an association.

Section 1542 of the California Civil Code
The ME SPA corporate office is based in California and Section 1542 of the California Civil Code states: “A general release does not extend to claims which the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release, which if known by him or her must have materially affected his or her settlement with the creditor or released party.” You hereby waive any right or benefit that you may have under this provision, or any similar provision of any other state’s law, and understand that you are giving up claims that you may not know or suspect to exist.

Severability Clause
If a provision of this Agreement is or becomes illegal, invalid or unenforceable in any jurisdiction, that shall not affect the validity or enforceability in that jurisdiction of any other provision of this Agreement; or the validity or enforceability in other jurisdictions of that or any other provision of this Agreement.

Policy on Misconduct
You understand that making any illicit or sexually suggestive remarks or advances during your visit will result in the immediate termination of the session and you will be held liable for the payment of the scheduled treatment. Furthermore, the therapists and employees reserve the right to refuse services to you for any reason that they deem necessary. See our Code of Conduct policy.

Security Cameras

For the safety and security of our guests, employees, and property, ME SPA may utilize video surveillance in public and common areas of the spa. Cameras are never located in treatment rooms, locker rooms, restrooms, or other private areas. By entering the premises, you acknowledge and consent to such recording.

Cancellation Policy
We have a 24-hour cancellation policy for individual appointments (48 hours for groups). If you miss, cancel, reschedule, or modify your appointment outside this designated time, you will be charged 50% of the total of your scheduled services. Late arrivals may result in a shortened appointment or rescheduling (charges may apply). To cancel or change your appointment, please call the spa directly.

Age Restrictions

*Guests ages 12-17 must be accompanied by a parent or legal guardian. A Minor Form must be completed by the parent or legal guardian before services are provided.
*Guests ages 12 - 15 must have a parent or legal guardian in the room while services are being performed.
*Children under 12 are not permitted in the spa.
*Facial Peels and HydroDermabrasion treatments are not available for minors.

*For purposes of these Terms & Conditions, 'ME SPA' includes Me Bath Spa Experience, LLC, its parents, subsidiaries, affiliates, successors, assigns, officers, directors, employees, contractors, agents and representatives.
